Discontinuation of Controlled Substance Licensing & Registration
Following are the steps to take if discontinuing registration for controlled substances:
- Notify Controlled Substance Program Coordinator within the UCUCA office.
- Send all remaining blank copies of Form 222 to the DEA.
- Transfer remaining inventory to another Departmental or Individual Registrant or contact OSEH for disposal.
- Arrange for record retention within your department or with UCUCA.
- Records should be kept for a minimum of three years post expiration date for State of Michigan controlled substance research licenses and DEA researcher registrations that are not renewed.
- DEA registrations and State of Michigan controlled substance licenses should be cancelled or transferred to a new location.
